Carpal tunnel syndrome also known as “CTS”, is the compression of the median nerve in the carpal tunnel. The carpal tunnel is a narrow passageway surrounded by bones and ligaments on the palm side of your hand. The median nerve is located on the palm side of your hand (also called the carpal tunnel). The median nerve provides sensation (ability to feel) to your thumb, index finger, long finger, and part of the ring finger. It supplies the impulse to the muscle going to the thumb thereby it’s compression will cause tingling, weakness and numbness on the side of your hand near the thumb.
What are the causes?
Any condition that increases the volume of the structure within the carpal tunnel or decreases the size of the tunnel will lead to compression of median nerve.
1. Acute Causes
-Sudden trauma -Fracture of the carpal bones i.e. Trapezium, Trapezoid, Hammate
-Crush injury-Crush injury of hand is common
2. Chronic Causes- It includes two types
Extrinsic cause- Tight gloves or cast
Intrinsic causes- Common in pregnancy
Ganglion or Cyst
Any infection i.e TB of wrist
3. Occupational Causes
-Prolonged use of power vibrating tools intense gripping
-Poor ergonomics while working with computer or keyboard
-Repeated wrist bending and extending
What are the signs and symptoms?
You feel weakness, numbness and burning sensation over the side of your hand near the thumb.
Night Pain is common in “CTS”.
You feel weakness in your thumb and inability to make fist.
Your performance is reduced in activities require gripping.
How to Diagnose?
Special Test to diagnose “CTS”-
Phalen Test- It is most widely used test to diagnose CTS, in this test you have to join back side of your palms together and keep it for “1 Minute”. If you experience numbness and tingling sensation over your hands, so the test is positive.
Reverse Phalen Test-This is reverse of phalen’s test, whole procedure is same but the position is reverse.
How to manage “CTS”?
-The first thing you can do if you have symptoms of Carpal Tunnel Syndrome, then use Cock-Up splint, to reduce pain.
– You should change your working pattern to subside the symptoms.
-Active Exercises of wrist and hand is to be done regularly within pain free range.
-Ultrasound, Laser and TENS can be used to reduce pain.
Prevention “To be kept in mind”
-Avoid Prolonged grasping of objects
-Use padded gloves while working with vibrating tools
-Take proper intervals between your work periods
-You have to avoid activities requiring repetitive hand and wrist functions
